Some stats (most of which aren't very reliable or meaningful due to sampling methods and sample size) I gleaned from the SLU failure reports:

2010 failure reports found - 24 (10991 made. Failure rate = 0.22%)
2011 failure reports found - 2 (5168 made. Failure rate = 0.04%)

Total failure reports found - 112 (27367 made. Failure rate = 0.41%)

0-5k - 0
5k-10k - 2
10k-15k - 3
15k-20k - 7
20k-25k - 6

35k-40k - 7
40k-45k - 9
45k-50k - 2
50k-55k - 3
55k-60k - 2
Unk/Other - 62

If we do a little more collapsing, it doesn't look like mileage has much to do with it:
0-10k - 2
10k-20k - 10
20k-30k - 11
30k-40k - 10
40k-50k - 11
50k-60k - 5
