I assume this was from a bad splicing job? I don't really understand why this happens... I've installed a few car audio systems in my time and I have never had to cut or tap the factory harness.

Bad splicing plus about a hundred other things, like connecting all ground wires to the head unit but not grounding the head unit. If a wire was too short they would just connect more wires till it was long enough, if a wire was too long it wouldn't be trimmed it would just be stuffed behind a pannel. And about a hundred other things that would just take too long to mention.
