Notchy shifting and feeling bumps while shifting in to gear

I've had the same issue where my transmission used to just slip into gears nice and smoothly and slowly over the last two years it's became increasingly more notchy and rough and now it's begun to very occasionally grind/lock me out when shifting at higher load/rpm. I've done everything short of either replacing the transmission or just selling the car at this point, it has irritated me enough.
