forgot to mention other maintenance items I've replaced on the car before hand....O2 upstream and downstream sensors have been replace, MAP sensor has been replaced, MAF sensors are still good and...

If the engine is running lean (as per codes you gave above), there are a lot of things that could be the problem - vacuum leak, bad injector, weak fuel pump, bad sensor, faulty ECM, high resistance...

I would first start with getting a fuel pressure gauge to be sure you don't have a fuel pump issue. The initial codes were MAF codes so if your fuel is good I'd go back to the MAF sensors. I know you...

My fuel pump hat broke off as well. It is very possible it has happened to OP as well, however I never had any of the issues OP posted about. I just had a messed up gas gauge but none the less still...
