Random failures happen - when you said it was still engaged even when depressed, it was obvious there was a lack of any fluidic pressure. Probably something as simple as

Random failures happen - when you said it was still engaged even when depressed, it was obvious there was a lack of any fluidic pressure. Probably something as simple as a bad seal in the slave.
