2) It's never happened to me.
3) This complaint seems so small, that I can't quite figure out what impact it has.

Yeah, my Tacoma X-Runner had a sunglass holder and 8 cup holders. WTF! I use my cup holder in the Z to hold my cell phone and I hold the cup with my hands..

I'm 6'1" so I would have loved to see a telescoping Steering wheel
