1. While you don't have to race to warp a rotor, it is rare for a DD.
2. 99.9% chance that this is the problem. A few 60-5 hard braking sessions may fix it. If that doesn't work, resurfacing the rotors may be the answer. As others have said, don't come to a complete stop after braking hard. If you do have to stop after hard braking, keep your foot off the brake - shift to neutral and/or use the parking brake if necessary.
3. Yep. If it only happens when braking, it not a balance problem.
