pop up the shifter console, drip the shifter shaft with a channel lock or pliers (I would use a rag around the teeth of the lock / pliers to not gouge the shifter), and hold it in place while twisting counter clockwise on the knob.

NOTE: you can actually damage the linkage to the trans by simply trying to unscrew and force the knob off without holding the shifter in place!

When you say pop up the shifter console, do you mean the silver trim around shift boot, or something else? Thanks!

Nevermind...I found a great picture here DIY Shift Boot Install (thanks e-licious!). I used channel locks on the shaft (with a towel around the shaft), and a plumbers style monkey wrench (again, with a towel) on the knob. A few hard turns and it was loose. Some minor indentations on the knob from the wrench are apparent. I'd use rubber next time.
