Under "high" setting, it barely warms up just the bottom and it quickly shuts off. The "low" setting keeps both the bottom and the back warm continuously until the switch is turned off.

Keep in mind that it turns itself off after awhile on 'high' setting. I think mine does that between 20 to 30 min. That could be the reason why you feel it wasn't warm enough. It doesn't do that...

Odds of both heating elements going out is pretty low so I'd look at the wiring and controls that they have in common. FSM will have wiring diagrams and other info.
