Default upper radiator hose failure?

Question for you mechanical inclined Z heads out there. Has anyone had any issues with upper radiator hose failure on our Z's? I installed a CSF radiator about 10k miles ago, and never had any issue, occasional replenish with coolant every oil change just to top off, but never an issue? What could have caused the failure? No visible tear on the hose.

My guess is I never upgraded my upper or lower radiator hoses? should I have upgraded hoses/clamps at install with the new radiator? I've heard that these stock clamps are poor quality also? Hopefully this is a quick fix, upgrading to mishimoto silicone hoses this weekend. Any help with this is appreciated.
