I'd first check the fan relay/socket connection. Unplug the relay and plug it back in.

If that didn't do any good, I'd check the voltage (or resistance) from the fuse to the fan, looking for a bad wire or connection.

Relay contacts may be bad, but the fuse should have protected them.
