A slightly more audible exhaust note and better exhaust flow. This exhaust and a tune is what makes up the HP/TQ difference between the base/sport and the Nismo.

For the tune, would you need to go to a Nissan-specific shop or could you go to any engine tuner?

The tune is from the factory. I suppose you could get a tune from Specialty Z with the exhaust but i would want to have more modifications done first. But you wouldn't necessarily need a tune with this exhaust, you'd pick up a few horsepower.

is the entire exhaust different than the base model, or is it just certain sections?

entire exhaust is different from the catalytic converter back. H pipe vs Y pipe, larger piping, better flowing.
